The Informal Support Clinical Assessment Protocol (CAP) (HC and CHA assessments) seeks to identify persons who have an informal support network that will benefit from having a “back-up” plan or alternative arrangement in place as the person declines over time. This CAP can help the assessor identify brittle informal support situations.
